DOSSIER & DOCUMENTS
Below are the documents that will need to be included
in your dossier. Several of the documents must be completed
in the forms provided by the agency. Some of the forms
carry time restrictions, so we would advise you to complete
them last. You will be given detailed instructions on
how to complete the dossier once you have applied to
the agency.
- Support letter from the appropriate social department
of dealing with International adoption
- Application for adoption
- Adopter(s) commitment letter
- Adopter(s) form for each applicant
- Copies of passports
- Marriage certificate
- Birth certificate for singles
- Home study
- Medical report for each applicant
- Employer's reference letter
- Police clearance (RCMP)
- Verification of residence
- Most recent photo pictures of each applicant including
family and home
